Section 1 : Listen to the attached audio file and as you listen, type the text that you hear. One way to do this is to download the file to your computer, then use
your computer’s default media player to listen to the file. That way you have all of normal recording buttons to use (Pause, volume, etc). The person who is
dictating the text is Dr. Karen Larsen, and she is dictating chart notes for patients she examined on October 11th.
Open a Word document and name it C5 Audio Transcriiption-xx. Create Chart Notes from the information below using the Vocabulary File and the other aids
listed below. When you have made the Chart Notes as complete and correct as you can, save the file, return to this assignment and submit it on this page.
You can earn up to 15 points for this assignment.
Use the Chart Note examples on page 193 to help you understand Dr. Larsen’s directions for paragraphs, colons, commas, etc. Other things you can use to
aid you are:
Online Medical Dictionaries and Medical Abbreviation lists
The list of medical abbreviations on pages 185-186 in the textbook
The list of medical terms and abbreviations I put together below.
After you have typed a first draft, go back through the text and make corrections to the format, spelling, and words. The sentences must make sense, and it is
sometimes hard to figure out where one sentence ends and the next begins, but do the best you can.
Don’t make yourself crazy – this is an OPTIONAL assignment. It does not have to be perfect. Unless you have already done this type of work, it is difficult. It
took me 4 hours to transcribe the file to create the printout version and none of you need to spend that amount of time stressing over this! Have fun with it,
do the best you can, and let yourself imagine what it would be like to do this type of work all day.
If you are hearing impaired or do not have headphones for your computer, you can use the printout below and create the chart notes from it. Believe me, it is
not any easier than listening to the audio file; in fact, you may want to rethink the notion that medical transcribing is a job you would like!

Section 2: 1. Read the following article, “But I Know the Alphabet,” which gives some additional information to the filing guidelines in the textbook on pages
204-209..
2. Then open the document Alpha_01.docx and use the Save As function to rename it Alpha_01-xx where the xx is your initials.
3. Alpha_01.docx contains several sections to review your filing skills, both alphabetical and numeric. Within each section, use the cut and paste functions to
re-arrange the items so they are in the correct order according to the instructions for that section. When you have finished, save your document and submit it
in this section.
