D​‌‍‍‍‌‍‍‌‌‍‌‍‍‌‌‌‍‍‍‍​IRECTIONS: Students must read attached case example about Clarence Jones and then write up a full 5A Assessment. Students may choose to make up additional details about Clarence’s circumstances and responses to questions to create a full assessment. Reference the 5A Model from the Hunter et al. (2016) book to guide you through what to write up for a functional assessment. The functional assessment write-up should be written professionally, succinctly, and in a narrative format such as “Mr. Jones appeared uncomfortable during assessment process. Mr. Jones reported his wife provides significant emotional support…” 1. ASSESSMENT: Students first write down a functional assessment that summarizes important information about the client and the presenting problem. Students will write down the nature of the presenting problem (yes, some information is listed in the case scenario, but you need to come up with more details), duration, and severity. Students will write down psychological, environmental, behavior factors that are relevant in the assessment. Students are encouraged to identify a screening tool or measure that may be utilized effe​‌‍‍‍‌‍‍‌‌‍‌‍‍‌‌‌‍‍‍‍​ctively for this patient 2. ADVISE. Based on the functional assessment students will write down what specific personalized treatment options they advise for this patient. Students should emphasize how the recommended treatment may decrease symptoms or improving functioning. 3. AGREE: Students will write a response from Mr. Jones regarding the treatment recommendations, noting any resistance to the treatment recommendations and how that resistance was resolved by the student. 4. ASSIST: In this section, students write out any actions taken to help Mr. Jones achieve his treatment goals. Students should list any information provided, skills taught, or other interventions delivered with a brief rationale for their choices. 5. ARRANGE: Summarize any plans for follow-up. The assessment should be between 1-2 pages in length, single spaced, in a 12-point Times New Roman or Ariel font. Pages should have standard 1-inch margins. EVALUATION This paper is graded on a 100 point scale. The grade will be based on student’s ability to fully answer the questions with an integrated understanding of the course material.
