DNP II: Week 2 Learning Activity #2 Codebook Guidelines and Grading Rubric
This assignment focuses on the development of a codebook. A codebook is used as a reference
for coding survey responses to prepare to enter the data into the analysis software. An example
of a pre-survey codebook is presented in Week 2. This assignment requires the student to
develop the post-survey codebook for this same study.
In completing this assignment, the student will:
1. CO #2: Employ methods of research using information technology to respond to gaps in
translation of best evidence to practice. (PO 1, 3, 4, 5, 9, 10)
2. CO #6: Apply basic statistical analysis methods to educational exemplars to contribute to
improved teaching and learning efforts supported by best evidence. (PO 1, 3, 4, 5, 10)
REQUIREMENTS:
1. To complete the assignment, students will need to access their course text book and
possibly the following databases: PubMed, CINAHL, MEDLINE, Cochrane Library, and
other resources in the Jay Sexter Library. These data bases can be accessed through the
Touro Online Library http://tun.touro.edu/jay-sexter-library/
2. The Codebook learning activity assignment is worth 50 points and will be graded using
the designated rubric. Grading criteria include quality of content, appropriate citations,
use of Standard English grammar, and overall organization and readability.
3. Create your assignment using a Microsoft Word application. The document should be
saved in a .doc or .docx format.
4. There is no required length but should be specific enough to address all requirements.
5. The following sections should be included in table format (follow the example provided
in Week 3):
a. Table column addressing the item
b. Table column addressing the variable code
c. Table column addressing the response codes
d. Table column addressing the compliance supporting codes
e. Highlighting of items to categorize domains

You will be developing your own code books based on instruments that you may use for your
own DNP project.
Instructions:
1) Review the General Self Efficacy Survey and General Self Efficacy Code book examples to
obtain an idea of how you will be structuring your own codebook.
2) Research possible instruments/tools that relate to your personal DNP project and select one to
develop your own codebook
3) Following similar formatting used for development of the General Self Efficacy Codebook,
construct your own codebook using your selected instrument.
4) Assign a variable code that can be identified by you as the project lead, and assign response
codes to each of the responses. Do not use 0 as a response code because this is typically used to
indicate lack of response. Last, highlight the items according to the domain or category
(demographic, knowledge, skill, attitude).
