2- Patient Care Study

GUIDELINES FOR PATIENT CARE STUDY
Course participants are required to select a patient whom he / she has nursed during the clinical practicum period and write a patient care study. It is expected that course participants will apply their knowledge in renal nursing to formulate a care plan using a problem solving approach.
Word limit: 3000 words
Suggested Format for Patient Care Study
1. Anonymity must be maintained; approval or consent must be obtained if required.
2. Cover page with
– the title of patient care study
– name of course participant (Mr. Wong)
– his/her parent hospital and seconded hospital (parent: Alice Ho Mui Ling Nethersole Hospital)
– number of words
3. Table of contents
4. Introduction / Learning objectives:
i. State the reason why you choose this patient (Patient was unable to shown he can manage peritoneal dialysis by himself even with repeated education)
ii. What do you expect yourself to achieve after completing this patient care study? (learning objectives)
5. Applied physiology and pathophysiology
– state the disease process and the management, briefly explain with related anatomy and physiology (detail
information can be submitted as appendix)
6. Patient care plan
– demonstrate holistic care with collaboration with other health care professionals whenever necessary
– formulate an individualized care plan using Nursing Process Approach
i. Patient’s profile:
– social demographic data (lives alone self-supported, poor hygiene)
– past health history (HT/DM/ESRF)
– clinical information e.g. reasons for renal consultation
(Patient was on peritoneal dialysis for 1 year and recurrent peritonitis occurred. Going to change to heamodialysis with tenknoff catherter removal.)
ii. Nursing assessment
– perform comprehensive nursing assessment including specialty specific assessment methods /
tools
iii. Nursing Diagnosis(es) / Identification of patient’s problems
– the identified problems or health issues should be evidenced by the data found in patient
profile / nursing assessment
1) Fluid Overload
2) Risk of infection (Internal Jugular Vein Cannulation)
3) Inbalance electrolytes
iv. Nursing care planning: Setting of goals and expected outcomes
– should be measurable, realistic and with time-frame
v. Implementation of care plan / nursing interventions
– describe appropriate interventions with rationales
– monitor the progress if having collaborated with other health care professionals
vi. Evaluation of care
– evaluate progress towards expected outcomes and reviews plans in accordance with evaluation
data
7. Related Pharmacology and procedures involved
– state the actions, uses and side-effects of the main types of drugs prescribed to the patient
– describe the nursing implications and the related health advice for patient
– if there is no drug prescribed to the patient, learner can demonstrate their knowledge by stating some
commonly used drug(s) for the disease(s) that the patient is suffering from.
8. Conclusion
– Conclude and make recommendation on the care process with regard to the particular patient or to the
specialty practice.
– Show evidence of reflective learning
9. References
– use a proper and consistent referencing style / format, preferably APA format
– include at least 5 different referencing sources
10. Appendix if any
