A. Discuss what you have found in your limited review for an evidenced based approach to this presenting problem.
B. Describe what you are selecting as having best potential to be effective in this situation. Provide rationale.
IV. Implementation of Intervention
A. Describe in some detail how you would implement the intervention.
V. Follow up Data collection and Data analysis
A. Describe when and how you will collect follow up data (Include Goal Attainment Scale in paper or in appendix)
B. Describe your findings based on the data analysis. Show your calculations.(in paper or in appendix)
