HSW 101 – Agency PaperGuidelines:
For this project you will be reporting on a social service agency: how it is organized, the mission statement, the services it provides, the population that the agency serves and any other interesting material you might find.
This approximately one to two page paper will report your findings of a Human Services agency exploration of your choice. You can find a list of human service agencies on-line by looking them up by category and location. Some examples of an agency would be those that provide services such as: drug and alcohol, mental health, adoption, foster care, homelessness, immigration services, etc. Make sure that the agency specializes in any area of child welfare and is a public agency. You can visit the website or call and ask to interview an individual who provides a human service in the agency.
The following points are a guideline for writing up your report. Please address as many as you can.
Agency Name
Population they serve
Their mission statement
An organizational chart if available (departments, department heads and workers)
The services they provide
The cost of their services
Requirement of services (ex. Insurance, Medicaid, low income, single parent, etc.)
How many staff do they employ
What are the credentials of the staff?
How is the agency funded: Does the agency receive any grant, county, state or federal funding.
Are there any issues that would force termination of services and what are they?
How long have they provided community services?
Any other pertinent information you would like to know about the agency.
This paper is about fact finding, creativity and self-reflection.

Understanding of Human Services and the public agency, Also use of academic language and Human Services vocabulary – 5 points
