Brief should include:

1.) A definition of target population (Women with breast cancer who live in rural communities) including the determinants of health that affect the population, comparing target population to similar populations at the local (Birmingham, AL ) state (Alabama) and national level using current data.
NOTE: The comprehensive comparison population is: women with breast cancer who live in the city/urban areas who have greater access to care. ****See Element: Description of the Population At Risk on the rubric attached.****

2.) An assessment of your community using a nursing conceptual or public health framework, identifying the strengths, needs and community resources of your target population. 3.) Two (2) community health diagnoses based on your assessment. Prioritize those diagnoses and provide rationale for such prioritization. Diagnoses must be aligned with appropriate Healthy People 2020 (HP2020) objectives. These sites may help with understanding community diagnoses:

Example of nursing diagnoses: 1.) Risk for breast cancer related to poor access to care and socioeconomic
barriers.

2.) Knowledge deficit related to risk factors as evidence by breast cancer

4.) Provide an explanation of the need for population approach in order to best address your identified community health diagnoses.

5.) Intervention proposal which addresses the needs of your target group. Identify if your intervention is aimed at primary, secondary and/or tertiary levels of prevention. EX: A community health fair at a local park which encompasses the targeted population women at risk for breast cancer. Women would receive free breast cancer screenings and education. For those who have breast cancer, community support groups could be implemented, etc.
