Population Health Big Data Assignment Instructions and Rubric
Go to the Agency for Health Care Research and Quality website found at www.ahrq.gov
From the blue toolbar at the top of the page, select “Data”, then choose “State Snapshots”
Choose your State from the map, and review the National Healthcare Quality and Disparities Reports. You will find a listing of the NHQR Quality measures for which data was collected in that state.
Choose a measure that is identified as “Far from Benchmark”, click on the link and review the summary data, graphs and charts provided.
You will write a four page paper (not including title page, references, or any appendices) analyzing a selected population based health concern from the table. Link the health concern to a relevant Healthy People 2020 (HP 2020), leading health indicators (LHI). Discuss the data from the HP 2020 “national snapshot” of this indicator as it relates to the selected health concern. Discuss the “evidence based interventions and resources” that have been identified for the LHI. Based on the literature, discuss why the HP 2020 LHI for this health concern not being met. What are the implications for this gap for members of your county/ community? What should be done to improve the outcome for this LHI? What is the role of the advance practice nurse in addressing this health concern and LHI? How could activities focused on improving meeting the leading health indicator be measured? What barriers to progress on these activities to you see and how could they be addressed?
